Community Recognition Endorsement Information Form

This form is to collect organization names for inclusion on the Community Recognition Award honoring Morton Sklar. We are inviting civil society organizations, community groups, and advocacy organizations to be listed as part of a collective to express our respect and gratitude for Morton’s dedication to the Khmer community. Organizations included will be acknowledged equally on the recognition certificate. Submission of this form confirms agreement.

Information submitted through this form will be used solely for the purpose of preparing the Community Recognition Award honoring Morton Sklar. Contact details will not be shared, published, or disclosed to third parties.
In‑country organizations will not be made publicly visible. The final, complete version of the recognition list will be shared only with Morton Sklar’s family.
