This form is to collect organization names for inclusion on the Community Recognition Award honoring Morton Sklar. We are inviting civil society organizations, community groups, and advocacy organizations to be listed as part of a collective to express our respect and gratitude for Morton’s dedication to the Khmer community. Organizations included will be acknowledged equally on the recognition certificate. Submission of this form confirms agreement.

ខាងក្រោមនេះជាទម្រង់ដើម្បីចុះឈ្មោះអង្គការដែលគាំទ្រការផ្តល់ពានរង្វាន់កិត្តិយសរបស់សហគមន៍ ទទួលស្គាល់ និងអរគុណលោកមេធាវី ម័រតុន សក្លារ ដែលបានជួយខ្មែរ។ យើងសូមអញ្ជើញអង្គការសង្គមស៊ីវិល ក្រុមសហគមន៍ និងអង្គការតស៊ូមតិទាំងអស់ ចុះឈ្មោះក្នុងបញ្ជីជាផ្នែកមួយនៃសមូហភាព ដើម្បីបង្ហាញពីការគោរព និងការដឹងគុណរបស់យើងចំពោះការលះបង់របស់លោក ម័រតុន ក្នុងការជួយសហគមន៍ខ្មែរ។ អង្គការដែលចុះឈ្មោះនឹងត្រូវបានទទួលស្គាល់ស្មើៗគ្នានៅលើវិញ្ញាបនបត្រទទួលស្គាល់នេះ។ ការចុះឈ្មោះជាកិច្ចព្រមព្រៀងយល់ព្រមរបស់សហគមន៍ ក្នុងពានរង្វាន់នេះ។

  • ទិន្នន័យរបស់សហគមន៍ដែលដាក់ជូនតាមរយៈទម្រង់នេះនឹងប្រើសម្រាប់តែការរៀបចំពានរង្វាន់នៃការទទួលស្គាល់របស់សហគមន៍ ផ្តល់កិត្តិយសដល់លោក ម័រតុន តែប៉ុណ្ណោះ។ យើងមិនចែកចាយ ចែករំលែក បោះពុម្ព ឬបង្ហាញ នូវព័ត៌មានលម្អិតទាក់ទងនឹងសហគមន៍ដែលចុះឈ្មោះ ដល់ភាគីទីបីឡើយ។

  • ដើម្បីសុវត្តិភាព យើងមិនបង្ហាញជាសាធារណៈឈ្មោះអង្គការក្នុងប្រទេសកម្ពុជាទេ។ ប៉ុន្តែជាចុងក្រោយបង្អស់ យើងនឺងដាក់ឈ្មោះអង្គការទាំងនោះក្នុងបញ្ជីនៃការទទួលស្គាល់នេះ ដែលនឹងចែករំលែកផ្តល់ជូនទៅគ្រួសាររបស់លោក ម័រតុន ផ្ទាល់តែ១គត់ប៉ុណ្ណោះ។
